Sponsorship Opportunities
SPONSOR AGREEMENT
Thank you for being a sponsor for the 4th Annual Navajo Nation Cancer Awareness and Advocacy Program (CAAP™) Cancer Conference Saturday, June 25th, 2011. This document represents an agreement between the Arizona Myeloma Network and _____________________________ for participation at the Conference which will be held at the Navajo Nation Museum at Window Rock, Arizona.
Please check the level of Sponsorship for which you are interested:
□ PLATINUM Sponsorship ($15,000) Advertisement in the event materials, and listed as a Donor on our website and in our newsletter distributed to cancer patients and organizations throughout Arizona. One 10’ table and prominent signage will be provided. Also, you will be listed as a sponsor for the Friday night, June 24th Faculty Reception and recognized as a host. Sponsorship tables and prominent signage will be provided at all events. You will also be listed as the Platinum sponsor of the Conference videos. You will be listed in all Pre-Conference activities, Entertainment and promotions, advertisement opportunities prior and post conference, including a full page ad in conference proceedings.
□ GOLD Sponsorship ($5,500-$10,000) You will be listed as a sponsor in the event materials June 25th, also on our website, and newsletter. One 10’Table and prominent signage will be provided. You will also be a sponsor for the Friday night, June 24th Faculty Reception. You will be listed as a sponsor for the Conference video.
□ SILVER Sponsorship ($3,000-$5,500) You will be listed as a sponsor in our June 25th Conference event materials, and also be provided with one 10’ table, signage and listed in our newsletter and on our website. You will also be invited to our Friday, June 24th Faculty Reception. You will be listed as a sponsor for the Conference video.
□ BRONZE Sponsorship ($1,500-$3,000) You will be listed as a sponsor for our Saturday, June 25th Conference and be provided with one 10’ Table and signage at the Saturday, June 25th event and invited to the June 24th Reception. You will also be listed on our website and in our newsletter.
□ COPPER Sponsorship ($1,000) You will be provided with a table for the Saturday, June 25th Conference and have a half page ad in the event materials. You will be invited to attend the June 24th Reception, and may bring your information/brochures to this event as well.
Additional sponsorships and donations are available:
Donation of $500: You will have a quarter page ad in our June 25th Conference event materials, and share a table to display your brochures/booklets at the conference.
Donation of $250: You will have a quarter page ad in our Conference materials.
Donations of any amount will be appreciated and you will receive a letter of acknowledgment and a tax receipt with our tax ID#.
Your support only allows us to provide this Conference at no charge to all participants.
To participate, the signed contract and full payment, with all information required for each sponsorship level, must be received prior to June 1, 2011.
